To enable us to fulfil our responsibilities as registered health care practitioners, we need to collect personal information. For example. your date of birth, residential address, occupation, marital status and number of children, your injury and/or illness history are also collected and recorded in your confidential patent history.
We use this information to gain an understanding of you as a patient and to enable your practitioner/s to provide helpful and accurate treatment recommendations. We are also legally bound by our registrative bodies and the Health Funds, to acquire this information.
We may also use this information to send you details of other products and services that may be of interest to you, for example, general clinic changes and updates. At any time, if you no longer wish to receive this additional information let us know and we will remove your details from our mailing list. The McLaren Street Clinic will under no circumstances sell or hand on your personal details to any other organisation, you can be assured that any mail you receive will have been sent to you direct from our clinic.
Other than basic contact details contained within your on-line enquiry and appointment request, we collect personal information in during your one on one private consultation.
Unless we are required to provide your personal information to others by law, by court order or to investigate suspected fraud or other unlawful activity, your information will only be seen or used by persons working in The McLaren Street Clinic.
Our information systems and files are kept secured from unauthorised access and our practitioners and contracted practitioners have been informed of the importance we place on protecting your privacy and their role in helping us to do this.
Subject to any legal restrictions, we would be happy to advise you what personal information we hold about you upon request. Personal patient information is never provided over the telephone. There may be some cost to you in providing this information if your request is complex or requires detailed searching of our records.
